Flexible Support During Periods of Transition

NDIS Participants can at times experience significant life events and periods of transition. Transition and life changes can include:

  • Moving home

  • Becoming a parent 

  • Entering a new relationships or experiencing a relationship breakdown

  • Moving into independent living or a different living setting

  • Developing greater mobility through taking up additional supports

  • Transitioning into or out of work

  • Transitioning into or out of other services including assisted housing, custodial services or hospital settings

VIA Care supports our participants through times of transition by making our delivery of core supports flexible and working with your needs. During period of transition we will work with you to plan ahead of time, support a care team approach and flex our supports around anticipated and emerging needs, in line with the wishes of participants. 

As guided by our client’s needs and wishes and working within the NDIS plan, we make sure that all the support we offer adapts to support any periods of change our clients go through. Some of the ways we ensure our delivery of core supports meets changing needs can include:

  • Pre-planning to clarify the participant’s goals immediately during and after the transition event and identify expected support needs and challenges throughout the transition. 

  • Flexible service delivery to maintain service continuity maintaining service continuity during transition 

  • Ensuring the participant has time to reflect on the changes and assess their post-transition support needs

  • Regular check-ins to assess the success of the transition and find ways to improve the support we provide in the new environment 

  • Taking a coordinated care-team approach with other supports and significant people in your life as directed by you

  • Staged approaches to stepping support up or down

  • Out-reach and arranging to work with participants in different locations throughout transition

  • Training support workers with the particular skills that are or will be required of them as a result of the transition
