Support Coordination

Support Coordinators help participants make the best use of their funded supports by organising and coordinating a range of services around the needs and lifestyle of the person being cared for.

Our support coordinators work with the participant to identify the supports they need and use their plan funding to help them achieve the best quality of life possible.

We can offer bi-lingual Support Coordination and are accustomed to working with professional and informal translators to make our participants feel comfortable and fully in control of the service they receive.


In addition to English speaking services we are fortunate to have Farsi/Persian speaking Support Coordinators and our Mandarin speaking office staff can provide communication support for all our Support Coordinators working with the Chinese-Australian community.


Our Support Coordinators can help participants:

  • Work through NDIS reviews to receive funding for services they need.

  • Ensure their plan is sustainably used and doesn’t run out of funds early

  • Plan their supports to promote overall independence and the achievement of goals

  • Manage changes of life circumstances such as moving into more independent or better supported living and adjust to different life circumstances such as becoming a parent or an empty-nester.
